We offer paperwork online so you can complete it in the convenience of your own home. Click on the link below, fill the form out, and submit it. Your information will be sent to a secure email and Morgan will have them ready for your appointment along with two forms that will need your signature.
For those whom are more comfortable, we also offer a printable form that you are welcome to print, fill out, and bring it in with you for your appointment!
